YOUTH ACTIVITIES: Someone attending a Safe Harbor youth activity must complete a Youth Permissions form and return it to the church office or the Youth Pastor.

EVENTS PLANNING: If a ministry is planning an event in the church that requires the help of the deacons to set up, please fill out one of these forms and bring it to the office, or put it in George Graff's mailbox at least two weeks prior to the event. Please fill out one of these forms even if you don't need deacon assistance. Copies of the form are also available in the church office.
